Chemokine Receptor와 Ligands

Chemokine(케모카인)은 몸 주변의 세포 움직임을 조절하는 고도로 보존된 (highly conserved) cytokine(사이토카인)입니다. 45개의 chemokine 계열은 모두 cysteine motifs(CC chemokine, CXC CX3C, XC)의 구조와 존재에 따라 4개의 subfamily로 나눌 수 있으며, 백혈구 표면에 주로 발견되는 7 transmemebrane domains이 있는 chemokine receptor, G-protein coupled receptor에 의해 인식됩니다. Chemokine이 chemokine receptor에 결합하면 receptor는 highly conserved DRYLAIV domain을 노출시키는 입체구조 변화가 생깁니다. 노출된 모티프는 인접한 G-protein에 결합하여 신호 전달 cascade를 작동시켜, 궁극적으로 세포가 분극화(polarise)하여 이동(migration)을 시작합니다.

일부 chemokine은 pro-inflammatory(전염증성)으로 여겨지며, 염증성 백혈구를 감염 부위 또는 조직 손상 부위로 모집하기 위한 면역 반응 중에 급격히 생산이 증가됩니다(CCL2, CCL3, CCL5, CXCL1, CXCL2, CXCL8). 반면, hemostatic chemokine(항상성 케모카인)은 정상적인 과정 동안 조직의 유지 또는 발달을 위한 세포의 이동을 제어하는 데 관여합니다. 여기에는 CCL19, CCL20, CCL21, CCL27, CXCL12 및 CXCL13이 포함됩니다. 염증의 측면에서, 포유류 chemokine family는 감염되거나 손상된 부위에서 수많은 inflammatory chemokine(염증성 케모카인)이 동시에 발현되도록 진화했습니다.

Chemokine receptor redundancy

면역 체계에 의해 감지되는 것을 피하기 위해 그들 자신의 'decoy(유인)' chemokine을 분해하거나 분비하는 능력을 진화시킨 병원체들이 있습니다. 다양한 다른 종류의 chemokine과 chemokine receptor를 발현함으로써, 면역체계는 병원체가 몇몇을 무력화시키더라도 감염 부위로 정확하게 이동하기 위해 다른 신호에 의존할 수 있습니다. 이로 인해 염증성 chemokine 시스템에서 명백한 redundancy(과잉,중복)이 발생했는데, 여러 ligand가 각 receptor에 결합하여 receptor를 활성화시키며, 각 receptor는ligand binding에서 promiscuity(난잡함)을 나타냅니다. 이는 선천성 면역과 관련된 진화적으로 가장 오래된 세포인 monocyte(단핵구)에서 가장 두드러지며, monocyte는 가장 넓은 범위의 chemokine에 반응할 수 있습니다. Chemokine 시스템의 redundancy, promiscuity 및 중복된 유전자 발현 패턴은 병원체에 의해 유발되는 chemotactic signal교란으로부터 보호하는 역할을 하지만, 이는 각 염증성 chemokine의 개별 역할에 대한 연구를 방해합니다. 실제로, 20년의 연구와 수백만 파운드의 투자에도 불구하고, 염증성 chemokine receptor에 대한 어떤 약리학적 길항제(antagonist)도 염증성 질환 치료에 사용 허가를 받지 못했습니다.

Chemokine receptor

Chemokine receptor는 서로 다른 종에 걸쳐 매우 높은 수준의 상동성(homology)을 공유하며, 이는 매우 오래 전 기원되었음을 시사합니다. 초파리, 성게, 멍게는 식별 가능한 chemokine이나 chemokine receptor를 가지고 있지 않지만, (G. gallus), 개구리(X. tropicalis), 제브라피시(D.rerio), 복어(F. rubripes) 같은 복잡한 유기체들은 모두 인간과 높은 유사성을 보이는 chemokine receptor를 공유합니다. 예를 들어 인간 CCR7(수지상 세포(dendritic cell)에 존재하는 chemokine receptor로 수지상세포가 림프절로 이동할 있게 함)는 유전자 시퀀스의82%를 CCR7, 80% 개구리, 64% 제브라피시와 공유합니다. 이러한 보존된 chemokine receptor의 돌연변이를 시간을 거슬러 추적함으로써, 일부 그룹은 조상 chemokine receptor의 기원을 척추동물 계통의 출현 시점인 6 5천년 전에서 5 6 4백만 사이로 추정하고 있습니다. 실제로, 이 시기는 신경관 조직(neural crest tissue), 폐쇄적인 순환 시스템, 혈액 기반 산소 운반 시스템, 조혈 시스템의 진화가 특징적인데, 이 모두는 일종의 통제된 primitive cell(원시세포)의 이동을 필요로 합니다.

Chemokine receptor 상동성

Chemokine receptor는 서로 다른 종에 걸쳐 보존되어 있을 뿐만 아니라, 서로 매우 유사하며 우리 게놈에 특이하게 위치하고 있습니다. 마우스와 인간 게놈 모두에서 두 개의 주요 CC chemokine 유전자군(gene cluster)과 개의 CXC 유전자군이 있으며, 두 종류 모두 다양한 non-clustered gene또는 mini-cluster gene이 존재합니다. 이 염색체 배열에 대한 설명은 게놈을 형성한 진화적인 힘에서 찾을 있습니다: 진화의 과정에서 유전자 중복(gene duplication)은 대부분의 gene family에 영향을 미치는 일반적인 사건입니다. 일단 중복이 발생하면, 개의 복사본은 독립적으로 진화할 있고 특수화된 기능을 발전시킬 수 있습니.

이로 인해 여러 개의 안전 보장 장치가 있는 복잡한 시스템이 만들어졌지만, 이는 연구자들이 개별 receptor의 역할을 밝혀내는 것을 거의 불가능하게 만들었습니다. 유전자 중복은 높은 상동성 유사한 구조를 가진 서로 다른 chemokine receptor(: CCR2 CCR1 서열의 51%, CCR3 78%, CCR5 71% 공유)라는 결과를 낳았고, 이는 flow cytometry나 immunohistochemistry를 위한 다른 chemokine receptor와 교차 반응하지 않는특정 항체를 개발하기 어렵게 합니다. 또한, 게놈에서 그들의 'clustering' compound KO mouse 만드는 것을 불가능하게 만드는데, 이는 유전들의 근접성(proximity)이 crossing-over(유전자 교환) 동안 유전자 재조합을 허용하지 않기 때문입니다(특히 CC inflammatory chemokine recptor인 CCR1, CCR2, CCR3, CCR5 모두 murine chromosome 9 같은 170kb tight cluster에서 발견됨).

백혈구의 chemokine receptor

Monocyte는 만성 염증성 질환인 RA15 (chemokine과 adhesion molecules16에 의해 매개되는synovial tissue(활막조직)와 synovial fluid(활액)에서 면역세포의 다량 침투가 특징)의 발병에서 주효 세포(effector cell)로 간주됩니다. 영향을 받는 관절의 monocyte 수는 일반적으로 임상 징후 및 증상과 관련이 있기 때문에, CCR2, CCR5, CCR1과 같은 chemokine receptor를 억제하여 염증이 있는 관절로 단세포가 모집되는 것을 차단하기 위한 여러 임상 시험이 시행되었습니다. CCR2와 CCR5 수용체 봉쇄는 RA 동물 모델에서 긍정적인 결과를 보였지만, chemokine 길항제를 사용하여 이러한 수용체를 차단하는 것은 RA의 영향을 받는 환자에게 효과적이지 않았습니다. In vivo 및 in vitro 연구에서도 CCR1 ligand 차단과 chemotaxis 억제 및 synovial inflammation 감소 사이에 양성 상관관계가 있는 것으로 나타났으나, CCR1 길항제 사용 임상시험에서 일부 그룹은 임상 개선 추세를 보였지만 다른 그룹은 전혀 효과가 없는 것으로 보고되는 등 상반된 결과를 보였습니다.

CCR1은 또한 다발성 경화증의 병리학에 역할을 하는 것으로 나타났습니다. CCR1의 ligand인 CCL3를 억제하면 급성 및 재발성 마비성 질환의 발생과 monocyte의 CNS28로의 침투를 예방하는 것으로 나타났습니다. CCR1결손은 생쥐의 다발성 경화증 모델에서 보호효과를 나타났으며, CCR1의 non-peptide agonist인 BX471는 급성 다발성 경화증의 rat 모델인 EAE에 대해 효과를 나타냈습니다. 이러한 초기의 유망한 결과로 화합물 BX471은 2004년에 다발성 경화증에 대한 임상시험에 들어갔고, 안전성에 대한 우려는 없었지만, 새로운 염증성 CNS 병변의 수를 감소시키지 못하는 것으로 나타나 임상 2상시험은 중단되었습니다.

Chemokine receptor의 임상시험

Chemokine receptor를 차단하는 임상시험이 효과가 없었는지에 대해, off target effect 부터 redundancy of the target까지 여러 가지 이유가 제안되었습니다.

처음에 CCR2 CCR5 공통 ligand를 공유하기 때문에 chemokine 시스템의 redundancy CCR2 또는 CCR5 길항제를 사용한 실험이 실패했다는 설명이 제기되었습니다. 그러나 chemokine receptor를 동시에 차단한 다른 실험에서 염증성 관절로 monocyte가 이동하는 것이 억제되지 않았으며, 이는 다른 chemokine receptor가 작용하고 있는 것을 시사하였습니다. 또한, CCR5 길항제 치료가 효과를 보이지 않는 것은 염증 부위에 대한 regulatoryT-cell(Treg)의 모집(recruitment) 억제로 설명 될 수 있습니다. T-reg 또한 CCR5 사용하여 이동하며, 활성 백혈구의 증식에 필요한 IL-2를 제거하고 항염증성 IL-10 TGF-b 방출함으로써 면역 반응을 억제하는 역할을 합니. 염증성 관절에 대한 Treg 모집 막음으로써, 과도한 염증은 monocyte 모집 감소의 긍정적인 효과를 상쇄할 수 있습니다.

길항제의 투여량도 CCR1 차단 시험 실패의 원인이 될 수 있습니다. 매일 10mg CCR1 길항제 MLN389 사용한 그룹에서는 임상적 효능을 보이지 않았지만, 다른 CCR1 길항제 CP-481715를 8시간마다 300mg 투약한 다른 그룹에서는 임상적 개선이 보고되었습니다. 이러한 결과는 높은 수준의 receptor 점유율을 보일 때에만 CCR1 봉쇄가 monocyte의synovial compartment로의 모집을 억제할 수 있다는 것을 시사합니다. 그러나 off targent effect 매우 높은 길항제 농도에서 관찰되는 중간수준의 보호효과를 설명하기 위한 잠재적 이유로 고려되어 왔습니다. 그러한 높은 수준(거의 길항제1g/)에서는 친화력이 낮은 다른 GPCR과도 효과적으로 교차 반응할 있을 만큼 충분한 약물이 존재합니다. 따라서 보고된 보호 효과는 이러한 다른 GPCR 통해 매개되었을 있습니다. Adenosine A dopamine D232 포함한 다른 GPCR 교차 반응할 있는 매우 강력한 human CCR1 inhibitor인 Comoound 1이 여기에 해당되었습니다. 화합물은 보호효과가 CCR1 억제가 아닌 도파민(T-cell 기능을 약화시키고 Th1d분비에 영향을 미쳐 다발성 신경증의 병태생리학에 관련됨)에 영향을 미쳐 나타나는 것으로 밝혀져 더 이상 개발되지 않았습니다.

질병의 heterogeneity

다발성 경화증(Multiple Sclerosis, MS)은 매우 이질적인 (heterogeneous) 질병의 예로, 뚜렷한 패턴의 neuron demyelination을 최소한의 구성요소로 가집니다. 이러한 다른 단계들은 다른 백혈구에 의해 특징지어지며 다른 chemokine receptor에 의해 발현됩니다. CCR1, CCR2, CCR5 CXCR3 모두 MS3 발병 및 진행과정에 관여하고 있습니다. 따라서 RA 유사하게, 임상 시험에서 효과를 나타내기 위해서는 질병을 가진 환자의 이러한 receptor의 발현과 활성화에 따라 이상의 receptor를 목표로 하는 것이 필요할 있습니다.

비록 연구들이 monocyte recruitment을 약리학적으로 변화시킬 있다고 제안했지만, 지금까지 설명된 예들은 chemokine 시스템의 복잡성과, 단일 receptor의 발현을 하향 조절함으로써 발생할 있는 결과의 다양성을 강조하고 있습니다. 중요한 chemokine=ligand 조합을 목표로 하여 염증성 질환 치료를 시도하기 전에, 다양한 chemokine receptor와 ligand가 어떻게 상호 작용하는지, 그리고 특정 백혈구들의 올바른 기능을 위해 무엇 필요한지 완전히 이해하고 결정해야 합니. 효과적인 약리학적 개입이 고려되기 전에, 각각의 염증성 chemokine receptor의 역할을 완전히 설명하기 위한 급진적인 새로운 접근법이 필요할 것입니.
